Welcome to VisionFirst!  Thank you for trusting your vision to the doctors of VisionFirst.  While an eye exam isn't painful, some people find it stressful, especially if it is their initial visit.  Remember, you are doing a smart thing by taking care of your eyes and keeping them healthy! 

When visiting VisionFirst on your initial visit, we ask your assistance with the following items:

1. Please complete and sign the new patient paperwork and bring it with you.  The three forms that must be completed are the Information/Health History form, the Patient Financial Responsibility form and the HIPAA form.  Click here to download the new patient forms.
2. Please bring your medical and/or vision insurance cards with you.  You will also need to know the policyholder's information such as employer and date of birth.
3. Please bring a list of your current medications with you.
4. Allow 60 minutes for your appointment.  Arriving a few minutes early will ensure you are seen promptly.
